Human Menopausal Gonadotrophin (HMG)
Gonadotrophins are used both for men and women to treat fertility problems since it helps women to ovulate and improve the sperm count of men.
Who are the beneficiaries?
Gonadotrophins are often given to women with polycystic ovarian syndrome and for women who have not responded to clomifene or have not become pregnant even though they responded to clomifene. Other possibilities include women with unexplained infertility and who are about to go for assisted conception treatment.
Gonadotrophins help men who have hormonal imbalance, especially for the one that originates in the pituitary gland or hypothalamus and that are linked to poor sperm quality, low sperm count, or poor motility.(Read more about 5 things you should do to produce high-quality sperm)
How does Gonadotrophins work?
Human Menopausal Gonadotrophins (hMG) is a combination of two vital hormones – follicle stimulating hormone (FSH), and luteinizing hormone (LH). These hormones are crucial for ovulation to take place and HMG directly stimulates the ovaries for the production of mature eggs.
Some of the drugs that are available now contain purified FSH to produce mature eggs and it causes little side effects. These drugs are given in combination with human chronic Gonadotrophins (HCG) so as to get better results.
Usually, the first course of injections will make your ovaries to start producing mature egg follicles. The HCG injections will help your ovaries to get the stability it needs for the production of healthy eggs.
What are the common brand names of it?
1.Repronex, Pergonal and Humegon that contains LH and FSH
2.Fertinex, Gonal F, Follistim and Metrodin that contains FSH
3.Perganyl that contains hCG
The duration of the treatment
Treatment will usually begin form the moment you are not ovulating and it will be done for seven to twelve days a month depending on the development and the time taken by your egg to mature. Your doctor will give you specific instructions about it and you do not need to come to the hospital every time and you can take it at your home itself.
The doctor will monitor the changes that are happening to your body and when you are likely to ovulate. So frequent trans-vaginal ultrasounds, frequent internal examinations, and blood tests are often necessary. As soon as the ultrasound shows that your eggs are mature, the doctor will give you HCG injection to trigger ovulation. Usually, 24 to 36 hours after HCG injection, ovulation occurs. You will be then advised to have sex after that or will be advised to have intrauterine insemination a day or two later.
The doctor usually recommends you to go through a maximum or three to six drug cycles. There will not be any success for it unless you follow the instructions of the doctor very cautiously. The doctor may increase the dose of the hormone or suggest another kind of treatment if you tried more than three times and yet not pregnant.
Whether there are any side effects?
Many women who have undergone the treatment have complained that it caused blurred vision, weight gain, fluid retention, bloating, and abdominal tenderness. Another problem with the treatment is that it is difficult for you to give the injection. However, the new Gonadotrophins that are purer cause less side effects and they are injected under your skin rather than into the muscles. It sure that it is an emotionally intense process and you need to have frequent monitoring.
Women have developed ovarian hyper stimulation syndrome (OHSS) in about three percent of cycles with hMG. This is a dangerous situation that is often signaled by the sudden onset of vomiting, severe pain, dehydration, nausea, weight gain, and headaches. Due to excessive number of eggs, the ovaries rapidly swell several times their usual size and it may result in fluid leak to the abdominal cavity.
If any these problems will come to you, then your doctor will advise you not to have sex or even to exercise strenuously mainly because of the risks of a high order multiple pregnancy. They are to be avoided since it creates a lot of complications in your body. If you want to improve fertility with no side effects, you can try some herb supplements.
What are the risks?
Multiple pregnancies and OHSS can happen even with the best monitoring and medical decision making. It is to be noted that the rates of multiple pregnancy is one in three with fertility drugs like hMG. So it is very important for you to consider the risks while attempting these treatments. One thing is sure that the use of hMG and the likes will not increase a woman’s risk of developing ovarian cancer.
What is the success rate of the treatment?
A number of studies have been done on the use of treatments using hMG and the likes, and they suggest that the about 75 to 85 percent women will ovulate with the help of these drugs and about 7 to 14 percent of them actually get pregnant.
The time in your cycle you have sex, your age and the speed, the quality and quantity of your partner’s sperm are the factors that affect your pregnancy rate. Before the use of these drugs, consult your doctor about all the odds.
